• Volunteers Rebate - Trust Beneficiary Statutory Declaration

  • Use this form to create a statutory declaration confirming your qualifying ownership interest in a property held by a trust.

    This declaration is required as supporting evidence for your Volunteers Rebate application and must be submitted alongside your completed application form.

  • What you'll need before you start

  • Have the following information ready before completing this form:

    • Address of each property you are applying for a rebate
    • Full legal name of the trust that holds each property, of which you are a beneficiary.
  • How to complete your statutory declaration

  • Step 1: Complete each field in the form.

    Step 2: Select Review to preview your statutory declaration. If you need to make changes, select Back to return to the form.

    Step 3: Once you have confirmed all information is true and correct, select Download to generate the printable document. The document will be created as a password-protected PDF. You can also select to have the PDF emailed to you, along with the password to open it. You will not be able to return to this form after this point, so please review carefully before proceeding.

    Step 4: Find an authorised person to witness your statutory declaration.

    Step 5: Meet with your witness and complete the following:

    • Say aloud: 'I, [your full name] of [your address], declare that the contents of this statutory declaration are true and correct.'
    • Sign and date the declaration in front of the witness.
    • Have the witness sign and record their name, address and qualification as a statutory declaration witness.

    Step 6: Attach your signed statutory declaration to your Volunteers Rebate application before submitting it.

    Important: If you do not provide a completed and valid statutory declaration, your application may be rejected.
